Family Forms

This Family Forms section provide a list of useful forms for customers to download and print out. Customers should note that these are formal Court forms and are provided in PDF format in order to preserve the layout of the original document.

Operation of the Family Forms System

The Family Forms system guides the user through their chosen form(s) via a series of questions that the user enters responses to.

Completion of Forms

Users may have to complete every step of a form. However, unneeded steps may simply be skipped.

Fees

Some forms require you to pay a fee. Your local court office can advise on current fees and can be contacted by calling NI DIrect on 0300 200 7812.

To decide which form you need, view the complete list of forms. A brief description of each form is detailed beside the form number.

You will need a set of forms for each person named as a respondent. Court staff can help you by explaining which forms you need to complete but cannot give you legal advice.

Documents and Details you may need

You may need the following information in order to complete your chosen form, but this will depend to a large extent on the form that you choose.

  • Any information that the Court has sent to you
  • Employment Details
  • Details of Income
  • Information about regular expenses and debts
  • Solicitors Address
  • Evidence of benefits received
  • Reference Numbers
