The LSANI Off-LAMS Booking Form to be completed by suppliers for all Off-LAMS requests. This form should be uploaded to LAMS when a LAMS application has been processed and a LAMS reference number is available.
This form must be sent to interpreters@lsani.gov.uk for completion.
